Why an Older Dog Starts Slipping on Hardwood Floors

The checklist I run before I say anything about “arthritis”
I don’t diagnose a slipping dog from the doorway. Before I say a word about joints, I want the owner to walk me through this list:
- When did it start — over a weekend, or has it crept in over months?
- Which end goes first — do the back feet slide out, or does the dog look unsteady all over?
- Does it happen only on hardwood/tile, or also on carpet and grass?
- Is there a sound — nails clicking differently, a dragging toe, a stumble on the turn?
- Any change in muscle — do the thighs or hips look thinner than they did six months ago?
- Pain signs — slow to rise, reluctant to jump on the couch, licking a joint, panting at rest?
- Weight and coat — has body condition shifted, and is the coat still full over the hips?
That list is the scene I want in my head before I touch the dog. Most of what follows is me unpacking why each item matters, and where the evidence actually stops.
The verdict, stated plainly
In the large majority of older dogs I see for this complaint, the slipping is a floor problem layered on top of a joint and muscle problem — not a neurological emergency. Hardwood strips away the traction that carpet and grass give for free, so a hip or stifle that’s already lost some strength and range of motion gets exposed the moment the dog steps onto a slick surface. That’s the boring, common answer. The uncommon answer — a neurologic issue such as degenerative myelopathy or vestibular disease — is real, but it has a different signature, and I’ll get to how I tell them apart.
I’ve watched this play out with a twelve-year-old Labrador named Gus, brought in by an owner who noticed his back feet “skating” every time he crossed the kitchen. It had started, she said, about eight weeks earlier — subtle at first, just a slight splay when he turned toward his water bowl, then progressing until he was visibly bracing before he’d cross the room at all. He was still fine on the backyard grass and completely normal on the living room rug. Carpet and grass supply extra grip, so a joint that’s lost some strength or range of motion can still get the dog across the room without an obvious stumble. Hardwood removes that margin. The dog’s compensations — a wider stance, a shorter stride, weight shifted forward off a sore hip — become visible the instant traction drops. That’s why owners so often tell me the dog is “fine outside” and only slides indoors: the floor isn’t causing the joint problem, it’s unmasking it.
Muscle loss compounds the effect. Once a joint is chronically uncomfortable, a dog naturally uses it less, and the muscle around that joint atrophies over weeks to months. Less muscle means less dynamic stabilization, which means more sliding on a slick surface — a feedback loop that’s worth interrupting early rather than waiting to see if it resolves on its own.

When the slip is not about the hip at all
A minority of my slipping cases turn out to be neurologic, and the pattern looks different from Gus’s. Degenerative myelopathy classically starts with the hind feet knuckling under — the top of the paw dragging on the ground — rather than a clean slide, and it tends to progress on every surface, not just hardwood. Vestibular disease usually comes on fast, over a day or two, often with a head tilt or eyes flicking side to side, and it doesn’t discriminate by flooring at all. If an owner tells me the onset was sudden, or that the dog looks unsteady on carpet and grass too, I move faster toward a full neurologic exam rather than assuming this is a hip story.

The exam that actually answers the question
